| Description | The Project is based on the desire to improve the conditions of asthmatic children. These children are frequently consulting the host of authorities and departments within the Danish Hospital Services, and often they experience the problems of a lacking communication between the primary and secondary care.
Using the the INTERNET technology the project will establish an 'Electronic Shared Patient Record’, which is the patient's case sheet, and which also provides a shared document for all the treating and consulting bodies and departments. By this we will achieve putting the patient in focus. Using the shared patient record the patient will be put in focus - including a direct access for the patient to review his own case information enabling the patient to communicate with professional treaters and fellow patients.
The shared patient record will hold all data relating to the individual patient and will be an electronic patient record. However, contrary to the normal scenario - with the hospital, the general practitioners and other bodies holding each their case record, they will all be using the one and same shared case record.
The shared patient record will allow cooperation and communication across sectors as all data are shared among the parties involved. This way the Asthma Centre will contribute to creating a coherent course of treatment for the individual patient, and using the INTERNET technology the patient will be provided with one rather than several individual approaches to treatment.
The coherent course of treatment is also expected to lead to a reduced consumption of resources with better results.
The INTERNET technology will open up to several scopes:
The children and their families will be given the possibility of consulting their own doctor or hospital for advice and guidance.
Families would be able to build a supporting network around the emerging 'technological site’. This network could consist of e.g. other families in similar conditions.
The system would enable a monitoring of the child. For example, the recording of entered data for lung capacity over time will allow the child to attend to its complaint in terms of any medication and diet. This will further the child's understanding of the relation between illness and life style.
In addition to selected patients, the project participants are The Pediatric Ward of the Hospital of Aalborg, the general practicioners, the Asthma School, the Health Insurance and the NetDoktor Company (developing the practical solution). In charge of the project is IT HEALTH CARE of the County of North Jutland. |
